FAIL! ipad 6 itunes restore fail 4013 error. no success probably hardware issue

Ipad 6 constantly restarting from apple boot icon
error code 4013 itunes.

Tried update/restore in ITUNES:
1. Tried restore mode
2. Tried DFU mode
3. Tried different cables
4. Tried different computers (3 total)

Final conclusion:

5. Hardware issue with USB port connector

6. Bad proximity sensor could be preventing reinstallation.
